Despite a sound record of fire safety, this Chicago nursing home has been rated as “much below average” by the Centers for Medicare and Medicaid Services after past inspections. It has, at times, had a number of health deficiencies more than three times the national average, and has received more than a dozen individual complaints and incidents within a three-year period.
Residents in Need
Medicaid investigations have found deficiencies in the care given and respect shown to Belhaven Nursing & Rehab Center’s residents in the past. Residents have, at times, reported clothing that had gone missing, while surveyors have noted that some areas of the home were markedly cluttered or visibly dirty. Reports have also noted that not all residents were receiving adequate assistance with hygiene, grooming, eating and drinking. These and other deficiencies were reported as corrected within a month.

In the past, Belhaven Nursing & Rehab Center has boasted a residency rate more than twice the national and Illinois averages. Because of this and staffing that has been previously deemed below average, residents received markedly less attention at this home than the national average. For example, in the past, individual residents have been reported to receive approximately 16 minutes of RN attention per day, which at the time was 30 minutes less than the national average and less than a third of the Illinois average.
