Boulevard Care Nursing & Rehab has been cited for safety and staffing deficiencies that earned it a “below average” rating from the Centers for Medicare and Medicaid Services in the past.

Fire Safety Citations

A past report found that this nursing home had 24 fire safety deficiencies — nearly seven times the national average at the time. These included the availability of escape routes for residents, a lack of a written evacuation plan and properly maintained smoke detectors. These deficiencies were reported as corrected the following month.

The Medicaid report included several reports of residents suffering preventable harm. For three consecutive years, residents reported actual harm because the nursing home either wasn’t free from accident hazards and risks or failed to provide supervision to prevent avoidable accidents. Other complaints include two instances of the home failing to either hire employees without a legal history of mistreating residents or to report and investigate reports of resident abuse, neglect or mistreatment.
