Quality measures make a determination of the overall quality of care that residents receive based on a grading rubric. When Salem Village Nursing & Rehab was investigated using these quality measures, it received a “below average” rating.

For example, the percentage of residents reporting moderate to severe pain was higher than national and Illinois averages, as were the percentages of patients that fell and experienced major injury, patients with UTIs and patients with catheters.
