Despite a much-above-average quality measure rating, Woodbridge Nursing Pavilion has been given a “below average” rating by the Centers for Medicare and Medicaid Services in the past. While the deficiencies found in this government report often represented a minimal level or harm or the potential for actual harm, the number of deficiencies and the number of residents affected by them brought the nursing home’s overall score down.

Emergency Preparedness

Investigations of this nursing home have, in the past, revealed that it was inadequately prepared for emergency situations, potentially placing many of the home’s residents at risk. For example, automatic sprinkler systems had not been properly maintained, nor were they supplied with enough water. The building also didn’t have automatic smoke detectors in the hallway or exits that were accessible at all times—all of these potential hazards would make a fire or emergency situation all the more dangerous.

Staffing Concerns

In the past, the reports filed by Medicare have indicated that Woodbridge Nursing Pavilion had a “much below average” staffing rating. During that time, the amount of time each resident received with a licensed nursing staff member has less than national and Illinois averages, and the time each resident received from an RN was less than half the national and Illinois averages.
